Gastric Bypass

Laparoscopic Roux-en-Y Gastric Bypass (LRYGB) is the most commonly performed procedure, and there is ample and robust evidence that it provides significant weight loss.

ROUX-EN-Y Gastric Bypass

It involves reducing the size of the stomach with the aim of reducing calorie intake and partially bypassing the small intestine, so that food does not pass through part of the small intestine as it travels through the digestive tract. There are many different techniques for LRYGB, all of which are acceptable. The operation is performed laparoscopically through 5 or 7 small incisions. The abdomen is inflated with carbon dioxide gas and a camera called a laparoscope and long surgical instruments are used. The surgeon operates by viewing the abdominal organs on a high-definition television screen. During LRYGB, part of the stomach is cut away to create a pouch-like pouch of about 30-60 cc in volume.

The remaining part of the stomach is left healthy, alive and connected to the small intestine. This so-called residual stomach has blood supply and produces digestive stomach acid. About 50 cm of the small intestine is then measured and cut off, and the long remaining part of the intestine is reconnected to the new, smaller stomach pouch using a stapler, hand suture or both.

The approximately 50 cm segment of small intestine at the end of the much larger gastric remnant is connected to the small intestine segment that extends into the smaller gastric pouch, approximately 80-150 cm further on. This prevents complete digestion of the food, because the food intake and digestive enzymes cannot mix with each other along a segment that is about 33% of the total length of the small intestine. This leads to weight loss.

On average, 60%-80% of excess weight is lost through this surgery. The LRYGB method is very effective because patients eat less and feel full quickly, and because they cannot fully digest and absorb the food they eat. Laparoscopic surgery reduces morbidity and shortens hospitalization compared to open surgery. Most patients are discharged after 2-4 days. On average, 60%-80% of excess weight is expected to be lost in the first 20 months after surgery. Most of the loss occurs within the first 6 months after surgery. This weight loss is made permanent with lifestyle changes such as diet and exercise in the postoperative period.

Advantages

Weight loss occurs faster than other procedures. Nothing is left in the body that can shift, break or disrupt its integrity. It has been proven to be a long-lasting surgical option for more than 30 years. Outcomes of total weight loss, long-term weight control and improved health status are better than in patients who choose other surgical procedures.

Disadvantages

The complication rate is slightly higher than other surgeries. The recovery period is longer. The patient has to take vitamin and mineral supplements for the rest of his/her life in the postoperative period. Dumping (rapid gastric emptying) syndrome may develop when sugary foods are consumed. This can cause diarrhea, nausea, cramps, cold sweats and chest pain.

Complications

Pulmonary embolism Deep venous thrombosis (DVT). Leakage from intestines. Abscess Hemorrhage. Intestinal obstruction. Stomach obstruction. Pneumonia (pneumonia). Wound infection. Urinary tract infection. Chronic nutritional problems. Protein deficiency. Vitamin and mineral deficiency. The risk of mortality in these surgeries is about 1 in 1000, depending on body mass index (BMI) and medical problems.
